《一念無明》的現實版悲劇

周日，留在家中備課，卻從電視新聞中看到一宗讓人心酸的倫常慘劇：一名男子殺母後跳樓自殺，重傷送院，其間曾表示「照顧老人家好辛苦！」

因照顧親人而觸發倫常慘劇接二連三

男子的母親是長期病患者，有糖尿病及腎病，不良於行，需要別人照顧。男子是街坊眼中的孝順仔，他是家中獨子，對體弱多病的母親一直不離不棄，更停止工作照顧病母，現時還聘用兩名外傭協助。懷疑他是未能承受照顧病母的壓力，趁周日外傭休假離家時殺母，再留下遺書由寓所跳樓自殺。其後，媒體報道男子被發現患有憂鬱病。

其實，懷疑因照顧年邁患病親人而壓力「爆煲」引發的倫常慘劇，今次已經是本年內第三宗！2月及6月的兩宗，分別是丈夫勒死腦退化老伴後跳樓亡，及丈夫勒死中風老伴後自首。類似慘劇接二連三發生，實在已為社會敲響警鐘！

男事主遭遇讓人想起電影《一念無明》

男子的遭遇，令人同情，亦無法不讓人想起之前的一套電影《一念無明》。片中的男主角余文樂，也是要獨力照顧患病的母親金燕玲，且堅持不把她送進老人院，最後心力交瘁，不單失去了工作、女友，更弄到自己出現情緒病，幾近走投無路。

當我看這套電影時，只感到莫大震撼，因為是那麼的真實，尤其是導演黃進，只是一個未夠30歲的小伙子，讓人驚訝他如何可以體會和理解這一切。

之前，家中也遇上老人問題，也為照顧老人家而苦惱和困擾過。老人家因為日漸年邁，身體和精神狀况也大不如前，且突然退化得很快，讓人有點措手不及。

照顧老人家殊不簡單

當時我曾經天真地以為，只要為雙親聘請外傭，問題便可以解決。但沒有想到，老人家的身體和精神問題，不是沒有經過護老專業訓練的外傭可以輕易應付得到。小至扶老人家起牀，因為不懂得借力，外傭很易扭傷；大至老人家的情緒問題，更非外傭可以應付和安撫得到，老人家往往會變得十分「忟憎」和躁動不安（我開始明白為何會有把老人家綁在牀上看似不人道的例子）。結果摩擦和衝突頻生，讓人苦不堪言。

後來，在無法可施的情况下，無奈要把老人家送進護老院。最先送進的那一間，所需月費不用1萬元，但環境、設施、對老人家的照顧都不大理想。後來再轉送去了現時那一間，環境、設施、對老人家的照顧都理想很多，但每月住院費卻高達3萬元。還幸自己也算是一個中產階級，再加上有兩個弟弟，兄弟3人分擔，也不至於太吃力。

切身遭遇讓我認識到的問題

在這個過程中，我體會和認識到幾個問題：

  • 首先，照顧老人家並不是一件簡單的事，並非一個普通人，又或者「靠晒」外傭，就可以輕易處理得到。那不僅需要很多知識和技巧，更需要莫大耐心。所以我完全明白余文樂在片中以及今次男事主說「照顧老人家好辛苦」那種挫折、沮喪和鬱結；
  • 第二，是否送老人家到護老院，是一個很艱難的決定。當子女因為工作，或家裏缺乏條件去妥為照顧，而把老人家交給受過專業訓練的人士去照顧，以及由設施齊備的院舍來支援，可能才是最好的方法。但過程中內心的痛苦掙扎、自責以及內疚，不足為外人道。所以我明白片中余文樂以及其他很多為人子女者，為何會如此固執地堅持，並會致以由衷敬意；
  • 最後，因為資源和人手所限，現時很多護老院的情况都十分不理想。如果要找較理想的護老院，所需費用就算是中產階級也會感到吃力。

所以，現屆政府暫時把施政重點放在年輕人身上，包括學業、就業、置業的所謂「三業」問題，這當然值得肯定，但也不能忽略長者，隨着香港逐漸步入老齡化社會，安老、護老問題，都是政府應該及早籌劃的。例如：

一、興建更多護老院，提供更多宿位；又或者

二、幫助那些不想住進護老院的老人家，做好居家安老支援服務，以及社區照顧；

三、在社會各種設施和服務中為老人家做好配套，讓他們也可以使用到。

少子化社會讓照顧老人家擔子更重

根據政府統計處在2015年發表的預測，未來人口將持續老化。統計處推算，撇除外籍家庭傭工，65歲及以上長者將由2014年的15%，顯著上升至2064年的36%，即是每3個有1個！人口老化將在未來20年最為急速，長者比例將在2024年升至23%，再在2034年進一步上升至30%，這與戰後嬰兒潮出生的人踏入老年有關。香港踏入老齡化社會的問題，已經迫在眉睫。

同一時間，香港的生育率卻持續低迷，由1981年每1000名女性有1933名活產嬰兒，下降至2014年只有1234名。簡單來說，就是每對夫婦由生育兩名子女下降至只生育一名，也就是所謂「少子化」社會的問題。

所以，將來香港往往是要由一名子女去照顧雙親，跟我們這一代可以由幾兄弟姊妹去分擔，完全不同，擔子也只會更重。

媽媽離世至今，轉眼已是九個年頭，她的容貌已在腦海中變得日漸模糊，但對她的懷念卻未有隨年日減退。

雖然媽媽患糖尿病超過三十年，有賴醫護的長期跟進和她服食藥物的紀律，一直未發生嚴重的迸發症，直至去世前一年多，無故在家中跌倒，自始便展開了年多進出醫院的痛苦歷程。

媽媽跌倒後，起初接受了盤骨接駁手術，但手術後卻出現爛腳問題，最終還是要切除右腳。她真的很堅强，接二連三手術的折騰和清洗傷口的痛楚，並沒能打擊母親的意志，印象之中，她不曾埋怨過苦難，呻吟過痛楚；我往探望她時，只會和我閒話家常，還催促我早點回家休息。

記得有天午後，好像是秋天時分，我趁午休往醫院探望她，她正在醫院內的小花園曬太陽；她坐在輪椅上，瞌上雙眼，嘴角掛着微笑，享受着温暖的陽光，看見我時還連連對我說：曬太陽很舒服。現在回想，還是未能分辨她堅强的求生意志是源於對美好大自然的不捨，抑或那根本就是大自然的一部分。

病榻中照顧母親的一年裡，令我經歷與她前所未有的親近。她出事的初期，還末找到工人，而醫院只能安排每週洗澡兩次，對於愛整潔的她，實在很難受。那時我便每週兩次到醫院幫她冲涼；那種感覺很奇怪－小時候媽媽替我洗澡，是自然不過的事；當我反過來為她做這事時，卻感到尷尬。幸好我們都愛說笑，這些不自然的情緒就在笑話和笑聲中被驅散了。

在母親進進出出醫院的一年裡，最害怕是醫院掛來的「午夜凶鈴 」，印象中曾接過兩三次這樣的電話，通知我們她情況危急，盡快趕往見最後一面，其中一次我還在醫院守候了一整夜，結果母親都安然渡過了。諷刺的是，我最終仍是錯過了她的最後一面；媽媽是在凌晨離世的，醫院發現她出事時已搶救無效。可幸是她在睡夢中離開，並無很大痛楚，面容也很安詳。在我心目中，也是當她睡着了，將來在天家再相見。

母親最後的日子，確實是有辛苦有疲累有擔心，但亦交織了很多與母親親暱相處的時光，這些片段成為了美好珍貴的回憶，讓我可在與媽媽暫別的歲月裡細細回味。

給母親的信

我腦裡亦時常想起事發經歷: 當日半夜收到謢養院通知妳在入院途中，當我到達醫院，妳已入了病房，見到在病床上的妳，我的淚水就如泉湧，因妳身上已插上許多醫療輔助器來維持生命﹗護士同時建議有什麼親人要到來見一面……；終於在一星期後，妳永遠離開我們幾姊弟了!

回想當初我們發現妳的行為和脾氣同以前有很大的分別，於是我們為妳奔走醫院和私家精神科醫生之間，最後的報告是當時的妳已患上中至晚期的腦退化症﹗所有的病徵都出現在妳身上，會漸漸忘記我們，見到妳所受的折騰，我內心更是非常的傷痛!
 
現在妳可以永遠別離痛苦，祝願妳在另一世界永遠享樂，永遠幸福。

我的媽媽

今年母親節很特別，因為有機會投稿，分享母親養育之恩情，特別媽媽患有認知障礙症已十多年，更有意思。

在2000年10月，我和母親到澳洲探望哥哥一家，期間發現母親十分健忘，回港後，瑪麗醫院老人科診斷為初期認知障礙症。隨着時間越長病情越深。到今天，媽媽對自己印象幾乎全都沒了。

從我到老人院接媽媽回家時，她看見我，第一句說話不是叫「仔仔」，而是叫「姐姐」，照顧媽媽十多年的我，難過之情，非筆墨可形容。

媽媽有七個仔女，我是排最細，自己覺得媽媽很本事，她除要照顧家庭，還要工作養家， 十分辛苦。媽媽廚藝十分了得，每逢節日她所煮的餸菜和糕點，真是數之不盡，美味可口。

雖然媽媽多年盡心盡力照顧家庭，不求回報，但家庭成員未必懂得欣賞、回報。還記得童年時，媽媽要入院割除子宮，完成手術後，回家休息，偶然在家中看見媽媽哭，她申訴爸爸沒有到醫院探望她，沒有人同自己出院，傷口又痛，這片段現在還記在我腦海中。

另外有一次吃晚飯，自己誤吞魚骨，但又吐不出而且帶血，痛得要哭。媽媽着急地背着我到醫院，終於魚骨在喉嚨中拑出。還有每逢仔女生病時，她總會悉心照顧，特別會煮粥或麥皮，務求仔女可以盡快康復，而且我自小多病，得到照顧特別多，感受特別深。

媽媽對仔女照顧和養育都不求回報，只希望仔女健康快樂。有句歌詞「父母恩，勝萬金」，母親節正是表揚母親偉大，提醒仔女應該對媽媽好好報答的日子。而我的心願是希望媽媽在生的日子都可以繼續由我照顧。我希望媽媽能有一個健康、快樂的晚年。
